CUSTOMER COMPLAINT AND FEEDBACK REQUIREMENT FOR MY SHOE STORE

 I have a shoe store named 'SPARX SHOES.
 In order to know my store performance,I need a feed back form from my customers. I also want to give them a chance to post their comments . I would also like to recieve an email as soon as I get any feedback. The basic details should contain the phone no. and the e-mail of the customer. Also, the shopping experience and the staff behavior. If the customer's need is satisfied and would they refer the store to anyone.
